Step 1: 

Register as an agent with CCBsure by clicking here.  (You must be registered as an agent with us to download our applications, "Submit Your Quote", or utilize our "Rating Calculator" and take advantage of our marketing "Tools and Resources").

Step 2:

Complete and return all required agent documents to Estelle Cummings:  Fax: 410-901-0830, email: Click here to Email Estelle.

• A Signed Agency Agreement – please fax the entire agreement not just the last page signed. Also, please note that the name on the agreement must be the same as the name that appears on your agency’s certificate of E&O coverage and license. If your agency operates under a DBA, please also indicate that name.

• A copy of the declarations page of your agency’s current E&O policy or a certificate of insurance for this coverage.

• A copy of your agency’s resident license.

• A completed W-9 form for your agency.

Step 3:

To secure a quote, please be sure the above has been submitted. In addition we will need:

• Completed applications
• Copy of prospect’s client contract or software licensing agreement
• Copy of prospect’s business promotional materials or website
